Blade to Blade is the fourth mission of Pinnacle of Fear and the primary objective is to escort Mithrax into the House of Fear territory on Neomuna so Mithrax can challenge The Undertaker to a duel.

- Defend Mithrax from the Vex
- Escort Mithrax
- View the Duel
- Eliminate the remaining House of Fear
- Fight The Undertaker
